Job Summary
Provide in-home physical therapy services across assigned Texas territory. Own the caseload as a case manager for patients' therapy plans, progress, and outcomes. Conduct comprehensive in-home assessments and skilled PT visits; develop and execute a plan of care (gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention); set measurable goals and monitor progress; drive multidisciplinary coordination with the care team; supervise PTA where appropriate; coach patients and caregivers with practical home programs; document thoroughly in Homecare Homebase; scheduled full-time field-based work with daily travel; requires active Texas PT license.
Required Qualifications
- Active Texas PT license (in good standing)
- Strong clinical judgment
- Comfort with daily travel across assigned territory
- Strong communication and documentation
