Job Summary
Conduct home health physical therapy segments to restore and improve patient functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations, establish individualized treatment and discharge plans, and perform therapy interventions using appropriate protocols. Ensure comprehensive clinical documentation is completed accurately within established deadlines and supervise assigned assistants. Maintain valid driver's license and hold required Washington and Oregon physical therapy licensure. Requires Doctor of Physical Therapy degree and CPR/BLS certification. Based in The Dalles, Oregon, Adventist Health Columbia Gorge has served the community since 1901.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations
