Physical Therapist (Home Health)

Conduct segments of treatment programs designed to restore physical, social, and mental functions while meeting department objectives. Serve as a co-leader in interdisciplinary approaches to patient groups. Perform therapy interventions using appropriate protocols, establish individualized care and discharge plans, and ensure communication with relevant parties. Complete evaluations to ensure quality standards, utilize clinical techniques to address assessment areas, and establish individualized treatment plans. Deliver comprehensive and accurate clinical documentation within established deadlines and supervise assigned assistants. Maintain a valid driver's license and hold required licensure in Washington and Oregon.
