Job Summary
Conduct segments of treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and perform therapy interventions using appropriate protocols. Provide comprehensive clinical documentation within established deadlines and supervise assigned assistants. Requires a Doctor of Physical Therapy (DPT) degree, state licensure, CPR/BLS certification, and a valid driver's license. Located in Stevenson, WA, this role supports Adventist Health Columbia Gorge's home health services since 1901.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations
- E-Verify participation acknowledgment
