Job Summary
Conduct home health and hospice therapy interventions using appropriate protocols to restore and improve physical, social, and mental functions. Plan and execute treatment programs while serving as a co-leader in interdisciplinary patient groups. Complete comprehensive clinical evaluations, establish individualized care and discharge plans, and ensure accurate documentation within established deadlines. Supervise assigned assistants and perform other job-related duties as assigned. Requires a Doctor of Physical Therapy degree, state licensure, CPR/BLS certification, and a valid driver's license. Located in Cornelius, OR.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations
- Acknowledgement of E-Verify Participation and Right to Work notices
