Physical Therapist (Home Health)

Conduct home health therapy interventions and establish individualized treatment and discharge plans for patients. Perform comprehensive clinical evaluations to ensure quality standards and complete accurate documentation within established deadlines. Serve as a co-leader in interdisciplinary patient groups and supervise assigned assistants. Maintain valid Physical Therapy licensure in applicable states, CPR certification, and a driver's license. This role is located in Garibaldi, OR, and requires a Doctor of Physical Therapy degree.
