Physical Therapist (Home Health)

Conduct segments of treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Perform evaluations to ensure quality standards, establish individualized treatment plans, and provide therapy interventions using appropriate protocols. Develop care and discharge plans, communicate them to relevant parties, and complete comprehensive clinical documentation within established deadlines. Supervise assigned assistants and perform other job-related duties as assigned. Requires a Doctor of Physical Therapy degree, state licensure, CPR/BLS certification, and a valid driver's license.
