Job Summary
Conduct home health and hospice physical therapy treatments to restore and improve patients' physical, social, and mental functions. Plan and execute individualized treatment programs, establish care and discharge plans, and communicate these protocols with appropriate parties. Perform comprehensive clinical evaluations, utilize appropriate techniques and tools, and complete accurate clinical documentation within established deadlines. Supervise assigned assistants and serve as a co-leader in interdisciplinary patient groups. Requires a Doctor of Physical Therapy (DPT) degree, state licensure, CPR/BLS certification, and a valid driver's license. Full-time position in Tillamook, OR, with Adventist Health's commitment to patient safety and vaccination requirements.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Access to and understanding of E-Verify Participation and Right to Work notices
