Job Summary
Conduct home health and hospice treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Perform evaluations to ensure quality standards, establish individualized treatment and discharge plans, and provide therapy interventions using appropriate protocols. Complete comprehensive clinical documentation within established deadlines and supervise assigned assistants. Requires a Doctor of Physical Therapy degree, state licensure, CPR/BLS certification, and a valid driver's license. Located in Forest Grove, OR, this role supports Adventist Health's personalized in-home care mission across California, Hawaii, Oregon, and Washington.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations
- Acknowledgement of E-Verify Participation and Right to Work notices
