Physical Therapist (Home Health)

Conduct home health therapy interventions using appropriate protocols and establish individualized treatment plans for patients. Complete comprehensive clinical evaluations, documentation within established deadlines, and develop discharge plans while communicating with appropriate parties. Serve as a co-leader in interdisciplinary patient groups and supervise assigned assistants. Maintain Physical Therapy licensure in all applicable states and hold current CPR certification. This role is based in Acampo, CA, with Adventist Health offering personalized in-home care across California, Hawaii, Oregon, and Washington.
