Physical Therapist (Home Health)

Conduct segments of treatment programs designed to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and perform therapy interventions using appropriate protocols. Ensure comprehensive and accurate clinical documentation is completed within established deadlines and supervise assigned assistants. Maintain a valid driver's license and hold required licensure in Washington and Oregon, including CPR certification.
