Job Summary
Conduct segments of treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and provide therapy interventions using appropriate clinical protocols. Perform comprehensive clinical documentation within established deadlines and supervise assigned assistants. Requires a Doctor of Physical Therapy (DPT) degree, state licensure, CPR/BLS certification, and a valid driver's license. Adventist Health locations span California, Hawaii, Oregon, and Washington, offering personalized in-home care and hospice services.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- All required vaccinations
- Acknowledgement of E-Verify Participation and Right to Work notices
