Job Summary
Conduct home health therapy interventions using appropriate treatment protocols and establish individualized care and discharge plans. Perform comprehensive clinical evaluations to ensure quality standards, develop individualized treatment programs, and complete accurate clinical documentation within established deadlines. Supervise assigned assistants and serve as a co-leader in interdisciplinary patient groups. Requires a Doctor of Physical Therapy degree, state licensure, CPR certification, and a valid driver's license. Located in Copperopolis, CA.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations
- Acknowledgement of E-Verify Participation and Right to Work notices
