Physical Therapist (Home Health)

Job Summary
Conduct home health and hospice therapy interventions using appropriate clinical techniques to restore physical, social, and mental functions. Plan and execute individualized treatment programs while serving as a co-leader in interdisciplinary patient groups. Complete comprehensive clinical evaluations, establish discharge plans, and ensure accurate documentation is submitted within established deadlines. Supervise assigned assistants and perform other job-related duties as assigned. Requires a Doctor of Physical Therapy degree, state licensure, CPR/BLS certification, and a valid driver's license. Based in Manzanita, OR, with Adventist Health's commitment to patient safety and vaccination requirements.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Acknowledgement of E-Verify Participation and Right to Work notices
