Physical Therapist (Home Health)

Conduct segments of treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and provide therapy interventions using appropriate protocols. Perform comprehensive clinical documentation within established deadlines and supervise assigned assistants. Maintain a valid driver's license and hold required Physical Therapy licensure for practice in applicable states. This role is located in Pacific City, OR, with Adventist Health offering personalized in-home care across California, Hawaii, Oregon, and Washington.
