Job Summary
Conduct segments of treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and provide therapy interventions using appropriate protocols. Perform comprehensive clinical documentation within established deadlines and supervise assigned assistants. Requires a Doctor of Physical Therapy (DPT) degree, state licensure, CPR/BLS certification, and a valid driver's license. Based in Oakdale, CA, this role supports Adventist Health's home health and hospice agencies across California, Hawaii, Oregon, and Washington.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Access to and understanding of E-Verify Participation and Right to Work notices
