Job Summary
Conduct segments of treatment programs to restore and improve physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Perform evaluations to ensure quality standards, establish individualized treatment and discharge plans, and provide therapy interventions using appropriate protocols. Complete comprehensive clinical documentation within established deadlines and supervise assigned assistants. Requires a Doctor of Physical Therapy degree, state licensure, CPR/BLS certification, and a valid driver's license. Position is based in Soulsbyville, CA, with Adventist Health's home health and hospice agencies serving the community across California, Hawaii, Oregon, and Washington.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations
- Acknowledgement of E-Verify Participation and Right to Work notices
