Job Summary
Manage and provide skilled physical therapy to patients with various diagnoses and disabilities in a home setting, adhering to physician treatment plans and individual patient needs. Perform patient evaluations and treatments while consulting professionally to maximize client independence based on their functional level. Work full-time day shifts in the home health environment, caring for diverse populations in their most comfortable locations.
Required Qualifications
- Bachelor's Degree From an accredited Physical Therapy Program
- Master's Degree From an accredited Physical Therapy Program
- D.P.T. From an accredited Physical Therapy Program
- National Provider BLS - American Heart Association
- Oregon Physical Therapist License
- 1 year Clinical experience in Home Health or healthcare setting
- active enrollment in the Providence postgraduate preceptor program
