Job Summary
Conduct home health physical therapy services as prescribed by physicians, assistants, or nurse practitioners, including regular or OASIS assessments and discharge planning. Evaluate patients using diagnostic and prognostic tests for muscle, nerve, joint, and functional abilities while instructing patients and families on device use and total therapy programs. Supervise assigned Home Health Aides and PTAs, observe clinical findings, and report patient reactions or condition changes to the health care team. Maintain HIPAA compliance, participate in QAPI programs, and ensure safety protocols during patient transfers and equipment evaluations.
Required Qualifications
- graduate of an approved school of physical therapist
- currently licensed and registered in this state
- at least one year of experience in physical therapy
- Able to speak, read and write in English
- Able to read assignments, follow directions
- Able to communicate and respond clearly on telephone and respond to patients spoken needs
- The ability to physically transfer, lift or assist patients whose average weight is 160 lbs with or without the aid of mechanical devices
- Able to spend 80% of the work standing and/ or moving about
- Able to walk, climb stairs, stoop, twist, bend and squat to perform essential job functions
- Is a graduate of a Physical Therapy school approved by the council of Medical Education of the AMA in collaboration with the National Boards of Physical Therapists or its equivalent
- Is licensed to practice in the State of Florida
- Has a minimum of one (1) year experience as a Physical Therapist
- Meets the health and physical requirements of the Home Health Care Agency
