Physical Therapist Home Health

As a full-time Home Health Physical Therapist, provide plan-of-care-based physical therapy services to homebound patients in North Cook County, delivering rehab in patients' homes, assessing physical function, developing and implementing rehabilitation goals, supervising PTAs and Home Health Aides, documenting observations and progress, and coordinating with physicians, the Clinical Manager, and care teams to optimize outcomes. Responsibilities include administering prescribed PT treatments, documenting evaluations and discharge summaries, and communicating progress to patients and families. Requires current physical therapy license, CPR certification, and ability to collaborate with a healthcare team; drivers license and vehicle insurance are required for this role.
