Job Summary
Conduct home health visits to adult and geriatric clients in the Concord, NH area. Perform diagnostic tests, implement integumentary protective interventions, and develop individualized care plans including manual therapeutic exercises and gait training. Continually assess and revise care plans while collaborating with nursing staff. Educate clients and family members on rehabilitative care and document observations using a touch pad tablet. Requires a current New Hampshire Physical Therapist license, one year of recent clinical experience, and graduation from an accredited program. Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- A current New Hampshire Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently
- Ability to manage time effectively
- Strong interpersonal skills
- Strong organizational skills
- Strong problem solving skills
- Solid computer skills
