Job Summary
Perform home visits in designated geographic territories to conduct comprehensive physical therapy evaluations, assessing mobility, strength, balance, gait, transfers, endurance, and safety risks. Develop individualized treatment plans to improve patient function and independence while providing therapeutic exercise, gait training, and evidence-based interventions. Collaborate with nurses and clinicians on a multidisciplinary team, educate patients and caregivers on exercises and disease management, and document care using an electronic medical record system. Full-time, part-time, and per-diem schedules available with daytime hours.
Required Qualifications
- Active and current license to practice as a professional Physical Therapist (PT) in the state of PA
- Ability to travel to cases as assigned
