Job Summary
Conduct home health visits to adult and geriatric clients in the Alexandria, VA area, performing diagnostic tests for mobility, strength, balance, and safety assessments. Implement individualized care plans including manual therapeutic exercises, gait training, and integumentary protective interventions for wound repair. Continually assess and revise care plans while collaborating with nursing staff, and educate clients and family members on rehabilitative activities. Accurately document all observations and interventions using a touch pad tablet. This PRN role supports senior living office clients with skilled nursing and rehabilitative care.
Required Qualifications
- A current VA Physical Therapist - PT license
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
Desired Qualifications
- Solid computer skills
- prior experience with electronic medical records (EMR) preferred
