Job Summary
Conduct in-home physical therapy assessments, develop individualized treatment programs, and guide patients and caregivers through therapy and device use. Document patient reactions and condition changes, then compile reports for staff and organizational development. Oversee physical therapy assistants and certified nursing assistants as needed. Provide local travel to patient homes within the Chicago area. Requires a Master's degree in Physical Therapy, valid Illinois license, and reliable transportation. Schedule includes Monday through Friday, 8:00 AM to 5:00 PM, with one weekend and two holidays monthly and annually. No on-call duties.
Required Qualifications
- Master's degree in Physical Therapy
- Valid Physical Therapist license in Illinois
- Valid Driver's License
- Reliable transportation
- Open to new graduates or candidates with less experience in physical therapy
- Local travel to patient homes within the Chicago area
- One weekend per month
- Two holidays per year
