Job Summary
Assess patient functioning levels, design individualized treatment plans, and document progress while educating patients and caregivers on therapy programs. Supervise support staff as necessary to ensure high-quality care delivery and participate in staff development activities. Perform local travel to residential homes for home health services. Schedule includes Monday through Friday from 8:00 a.m. to 5:00 p.m., with one weekend per month and two holidays; no on-call rotation required. A valid Illinois PT license, Master's degree, and minimum one year of PT experience are required, with one year of home health experience preferred. Familiarity with OASIS and HCHB is also preferred.
Required Qualifications
- Valid Illinois PT license
- Master's degree in Physical Therapy
- Minimum 1 year of PT experience (no new grads)
- Experience in a Healthcare Setting
- Regular Driver License (DL)
- Monday-Friday; 8:00a/9:00a-5:00p + 1 weekend per month (Saturday & Sunday; day shift w/scheduled patients); 2 holidays (1 summer / 1 winter); no on-call
- Local travel to residential homes
Desired Qualifications
