Job Summary
Conduct home health physical therapy treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and provide therapy interventions using appropriate protocols. Perform comprehensive clinical documentation within established deadlines and supervise assigned assistants. Maintain active licensure in Washington and Oregon, hold a valid driver's license, and possess current CPR or BLS certification. Work full-time in a 49-bed hospital and home health setting serving the Columbia River Gorge community.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations as a condition of employment and annually thereafter
