Job Summary
Conduct segments of treatment programs to restore and improve physical, social, and mental functions while meeting department objectives. Serve as co-leader in interdisciplinary patient groups, complete evaluations to ensure quality standards, and establish individualized treatment, care, and discharge plans. Provide therapy interventions using appropriate protocols, supervise assigned assistants, and complete comprehensive clinical documentation within established deadlines. Maintain Physical Therapy licensure in all applicable states, including Washington and Oregon, along with CPR/BLS certification and a valid driver's license. Work in the home health setting within Adventist Health Columbia Gorge's 49-bed hospital and medical office network serving the Columbia River Gorge community.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- At least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations
- E-Verify participation acknowledgment
