Physical Therapist (Home Health)

Conduct segments of treatment programs designed to restore and improve physical, social, and mental functions while meeting department objectives. Serve as co-leader in interdisciplinary patient groups, complete evaluations to ensure quality standards, and establish individualized treatment, care, and discharge plans. Provide therapy interventions using appropriate protocols, supervise assigned assistants, and complete comprehensive clinical documentation within established deadlines. Maintain valid Physical Therapy licensure in applicable states, a Doctor of Physical Therapy degree, and required CPR/BLS certifications. Work in a 49-bed hospital and home health setting in The Dalles, Oregon.
