Job Summary
Conduct home health therapy interventions using appropriate protocols to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and communicate them to appropriate parties. Perform comprehensive clinical documentation within established deadlines and supervise assigned assistants. Requires a Doctor of Physical Therapy degree, state licensure, CPR certification, and a valid driver's license. Adventist Health locations span California, Hawaii, Oregon, and Washington, offering personalized in-home care with many recipients of Home Care Elite awards.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Acknowledgement of E-Verify Participation and Right to Work notices
