Physical Therapist (Home Health)

Conduct segments of treatment programs to restore and improve physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Perform evaluations to ensure quality standards, establish individualized treatment and discharge plans, and provide therapy interventions using appropriate clinical techniques. Complete comprehensive and accurate clinical documentation within established deadlines and supervise assigned assistants. Maintain valid Physical Therapy licensure in applicable states, CPR/BLS certification, and a driver's license. This role is located in The Dalles, Oregon, at Adventist Health Columbia Gorge, a 49-bed hospital and home health provider serving the Columbia River Gorge community since 1901.
