Physical Therapist (Home Health)

Complete evaluations to ensure quality standards and establish individualized treatment plans for patients. Provide therapy interventions using appropriate protocols and create discharge plans communicated to relevant parties. Supervise assigned assistants and perform other job-related duties as assigned. Ensure comprehensive and accurate clinical documentation is completed within established deadlines. Maintain Physical Therapy licensure in all applicable states, including Washington and Oregon, along with required CPR certification and a valid driver's license. This role supports Adventist Health Columbia Gorge's mission to serve the Columbia River Gorge community since 1901.
