Job Summary
Conduct home health and hospice therapy interventions using appropriate clinical techniques and protocols to restore physical, social, and mental functions. Plan and lead treatment programs that serve the community, while serving as a co-leader in interdisciplinary patient groups. Complete comprehensive evaluations, establish individualized care and discharge plans, and ensure accurate clinical documentation within established deadlines. Supervise assigned assistants and perform other job-related duties as assigned. Requires a Doctor of Physical Therapy degree, state licensure, CPR/BLS certification, and a valid driver's license. Adventist Health locations span California, Hawaii, Oregon, and Washington.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- All required vaccinations as a condition of employment
- E-Verify participation acknowledgment
