Physical Therapist (Home Health)

Conduct treatment programs designed to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Perform evaluations to ensure quality standards, establish individualized treatment and discharge plans, and communicate these plans to appropriate parties. Provide therapy interventions using appropriate protocols, supervise assigned assistants, and complete comprehensive clinical documentation within established deadlines. Maintain a valid driver's license and hold required licensure in Washington and Oregon. This role is located in The Dalles, Oregon, within Adventist Health Columbia Gorge's home health services.
