Job Summary
Conduct home health treatment programs to restore and improve physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and communicate these plans to appropriate parties. Provide therapy interventions using appropriate protocols and supervise assigned assistants. Maintain comprehensive and accurate clinical documentation within established deadlines. Hold a Doctor of Physical Therapy degree, state licensure, and valid driver's license.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations as a condition of employment and annually thereafter, where applicable
- Acknowledgement of E-Verify Participation and Right to Work notices
