Job Summary
Conduct physical therapy interventions and complete evaluations to restore patients' physical, social, and mental functions. Establish individualized treatment and discharge plans, ensuring accurate clinical documentation is completed within established deadlines. Serve as a co-leader in interdisciplinary patient groups and supervise assigned assistants. Maintain Physical Therapy licensure in applicable states, including WA-PT and OR-PT, along with BLS/CPR certification and a valid driver's license. Work in the home health division of Adventist Health Columbia Gorge, serving the Columbia River Gorge community.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations as a condition of employment and annually thereafter
