Physical Therapist (Home Health)

Conduct segments of treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and provide therapy interventions using appropriate protocols. Perform comprehensive clinical documentation within established deadlines and supervise assigned assistants. Maintain valid Physical Therapy licensure in applicable states, CPR certification, and a driver's license, with vaccinations required as a condition of employment. This role supports Adventist Health Columbia Gorge's community services in The Dalles and surrounding areas.
