Job Summary
Conduct home health physical therapy segments to restore and improve patients' physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and provide therapy interventions using appropriate clinical protocols. Perform comprehensive clinical documentation within established deadlines and supervise assigned assistants. Requires a Doctor of Physical Therapy (DPT) degree, state licensure, and valid driver's license. Based in The Dalles, Oregon, Adventist Health Columbia Gorge has served the community since 1901.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations as a condition of employment and annually thereafter, where applicable
- Acknowledgement of E-Verify Participation and Right to Work notices
