Job Summary
Conduct home health therapy interventions and establish individualized treatment plans for patients while completing comprehensive clinical evaluations. Serve as co-leader in interdisciplinary patient groups and supervise assigned assistants. Ensure all plans are communicated to appropriate parties and maintain accurate clinical documentation within established deadlines. Requires a Doctor of Physical Therapy (DPT) degree, state licensure, CPR certification, and a valid driver's license. Based in Forest Grove, OR, this role supports Adventist Health's home health and hospice agencies across California, Hawaii, Oregon, and Washington.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Access to and understanding of E-Verify Participation and Right to Work notices
