Physical Therapist (Home Health)

Job Summary
Conduct segments of treatment programs to restore and improve physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and perform therapy interventions using appropriate protocols. Deliver comprehensive clinical documentation within established deadlines and supervise assigned assistants. Maintain valid Physical Therapy licensure in applicable states, including Washington and Oregon, along with CPR certification and a valid driver's license. This role is located in The Dalles, Oregon, within Adventist Health Columbia Gorge's home health services.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- Age of at least 21 years
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR certification
- Washington-Physical Therapist (WA-PT) licensure
- Oregon-Physical Therapist (OR-PT) licensure
- All required vaccinations
- Acknowledgment of E-Verify Participation and Right to Work notices
