Job Summary
Conduct treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations, establish individualized treatment and discharge plans, and perform therapy interventions using appropriate protocols. Provide comprehensive clinical documentation within established deadlines and supervise assigned assistants. Maintain valid Physical Therapy licensure in Oregon and Washington, along with CPR certification and a driver's license. This role supports Adventist Health Columbia Gorge's community services in The Dalles, Oregon.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- At least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations
- E-Verify participation acknowledgment
