Physical Therapist (Home Health)

Conduct segments of treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and provide therapy interventions using appropriate clinical techniques. Perform comprehensive and accurate clinical documentation within established deadlines and supervise assigned assistants. Maintain a valid driver's license and hold required Physical Therapy licensure in applicable states, including Washington and Oregon. Work in a 49-bed hospital and home health setting in The Dalles, Oregon, as part of Adventist Health Columbia Gorge.
