Job Summary
Conduct treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations, establish individualized treatment and discharge plans, and perform therapy interventions using appropriate protocols. Supervise assigned assistants and ensure all clinical documentation is comprehensive and accurate within established deadlines. Hold a Doctor of Physical Therapy (DPT) degree with licensure in Oregon and Washington, along with CPR/BLS certification. This role is based in The Dalles, Oregon, at Adventist Health Columbia Gorge, a 49-bed hospital and home health provider serving the Columbia River Gorge community since 1901.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- At least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations
- E-Verify participation acknowledgment
