Physical Therapist (Home Health)

Job Summary
Conduct home health and hospice therapy interventions using appropriate clinical techniques and protocols. Plan and execute treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete comprehensive clinical evaluations, establish individualized care and discharge plans, and ensure accurate documentation is submitted within established deadlines. Supervise assigned assistants and perform other job-related duties as assigned. Requires a Doctor of Physical Therapy degree, state licensure, CPR/BLS certification, and a valid driver's license. Adventist Health locations span California, Hawaii, Oregon, and Washington.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Access to and understanding of E-Verify Participation and Right to Work notices
