Physical Therapist (Home Health)

Conduct home health therapy interventions using appropriate clinical techniques and tools to restore and improve physical, social, and mental functions. Complete patient evaluations, establish individualized treatment and discharge plans, and ensure plans are communicated to appropriate parties. Supervise assigned assistants and perform other job-related duties as assigned while maintaining comprehensive clinical documentation within established deadlines. Serve as a co-leader in interdisciplinary approaches to patient groups. Requires a Doctor of Physical Therapy (DPT) degree, state licensure, and valid driver's license.
