Physical Therapist (Home Health)

Conduct segments of treatment programs designed to restore and improve physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and communicate these plans to appropriate parties. Provide therapy interventions using appropriate protocols, supervise assigned assistants, and complete comprehensive clinical documentation within established deadlines. Maintain a valid driver's license and hold required licensure in applicable states.
