Physical Therapist (Home Health)

Conduct segments of treatment programs to restore and improve physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and perform therapy interventions using appropriate protocols. Provide comprehensive clinical documentation within established deadlines and supervise assigned assistants. Maintain Physical Therapy licensure in applicable states, hold a valid driver's license, and possess required certifications including CPR/BLS and state-specific practice licenses.
