Job Summary
Conduct segments of treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Perform evaluations to ensure quality standards, establish individualized treatment and discharge plans, and provide therapy interventions using appropriate clinical techniques. Complete comprehensive clinical documentation within established deadlines and supervise assigned assistants. Maintain a valid driver's license, hold required state licensure (WA-PT, OR-PT), and possess CPR or BLS certification. This role supports Adventist Health Columbia Gorge's home health services in The Dalles, Oregon, within a 49-bed hospital and medical office network.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations as a condition of employment and annually thereafter, where applicable
- Acknowledgement of E-Verify Participation and Right to Work notices
