Job Summary
Conduct home health therapy interventions using appropriate protocols to restore physical, social, and mental functions while establishing individualized treatment and discharge plans. Complete comprehensive clinical evaluations, ensure quality standards, and perform accurate documentation within established deadlines. Supervise assigned assistants and serve as a co-leader in interdisciplinary patient groups. Requires a Doctor of Physical Therapy (DPT) degree, state licensure, CPR/BLS certification, and a valid driver's license. Full-time role based in The Dalles, Oregon, with mandatory vaccinations and E-Verify participation.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations
- E-Verify participation acknowledgment
