Physical Therapist (Home Health)

Conduct segments of treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and communicate these plans to appropriate parties. Provide therapy interventions using appropriate protocols and supervise assigned assistants. Deliver comprehensive and accurate clinical documentation within established deadlines. Maintain valid Physical Therapy licensure in applicable states, including Washington and Oregon, along with required CPR/BLS certification and a valid driver's license.
