Job Summary
Conduct home health physical therapy treatment programs to restore and improve patient functions while serving as a co-leader in interdisciplinary patient groups. Perform evaluations, establish individualized treatment and discharge plans, and ensure comprehensive clinical documentation is completed within established deadlines. Supervise assigned assistants and perform other job-related duties as assigned. Requires a Doctor of Physical Therapy (DPT) degree, state licensure, and valid driver's license.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- AHA-BLS or ARC-BLS or AHA-CPR or ARC-CPR
- Washington-Physical Therapist (WA-PT)
- OR-PT
- All required vaccinations as a condition of employment
- E-Verify participation acknowledgment
